Case Medical Plan for Students

All students registered for one or more credit hours for fall and spring semesters will be automatically billed for the Case Medical Plan. For information on costs and coverage please go to http://studentaffairs.case.edu/medicalplan/ or contact the University Health Services at (216) 368-2450.

Students who have insurance comparable to the Medical Plan and do not want the Case medical coverage must waive coverage each fall and spring semester. Things you might want to consider:

That your alternative coverage is ADEQUATE to meet your health needs. Medical care in the US and in Cleveland is expensive. If you have questions, Louise Matchett ( lmb18@case.edu ) or Mary Beth Katitus ( mtk12@case.edu ) can answer them.

Many commercial insurance plans do not cover a student after a certain age (you would need to check this age limit for each plan); the Medical Plan must be waived each semester (fall and spring) that you do not wish this coverage (if you take the coverage in the spring, it automatically covers you through the summer).

The Medical Plan waiver process is now online (paper based waivers have been eliminated).

Students who waive the plan for a given semester are eligible to apply for coverage during that semester if they experience a termination of their current medical coverage that is beyond their control. An Optional Dependent Medical Plan is available to dependent spouse, domestic partner, or children of students who are covered under the Case Medical Plan. University Health Services

The University Health Service is the student's primary health care facility on campus.  Students are seen by appointment for all their health care needs.  These appointments are available on a same day basis for urgent needs. 

Medical Forms

All Case students are required to submit a Personal Information and Immunization Record whether or not they plan on using the University Health Service.  You can download the New Student Medical Packet . You can print them out, complete the forms and mail them to the University Health Service.  Students in the schools of Medicine, Nursing and Dentistry may be asked to send a copy of their immunizations to the school, this does not replace the need to send the information to the University Health Service.

If you are enrolling for the fall semester, we ask that you complete and return the forms by July 1 . For the spring semester, the forms should be returned by January 1. If you are unable to meet these deadlines, please submit the forms as soon as possible.

If you are having difficulty completing the Immunization Record, we will accept a copy of you high school immunization record attached to the Immunization Record. Please submit whatever information you have, we will review it, and let you know if there are any deficiencies.

You may fax the form to us at 216-368-8530 but we will not mail the information until we have the original request mailed to us. We require a minimum of 4 working days after we receive the form to process it. We will not fax medical information for confidentiality reasons. A tuberculin screening on campus is required of all new International Students.  All international students wishing to have a Tuberculin Skin test should come to the University Health Service to get this done. The Health Service is open Monday - Friday 8:30 a.m. - 4:40 p.m., with the exception of Thursday when they open at 9:30 a.m.

University Counseling Service

University Counseling Services (UCS), a division of Student Affairs, offers help to students who experience a variety of difficult personal and interpersonal challenges. Our office is staffed with psychologists, psychiatrists, social workers, counselors, and doctoral-level counseling trainees who specialize in helping students adjust to these challenges and to college life in general.

Athletics, Facilities, Physical Education, & Intramurals

The Physical Education Department is here to serve the recreational and leisure needs of the students, faculty, and staff at Case Western Reserve University . Diversified programs and experiences are offered throughout the year.
