Temporary Change in ParkingThe university understands that life events sometimes necessitate a member of the Case campus community to park in an area other than his/her assigned lot. In order to request a temporary change in parking, please follow the instructions below:
1) First, request a note from your doctor specifying your injury/condition that is causing your need for parking closer to your office/work area and the length of time that you will need the temporary parking. A faxed copy is acceptable. Example of possible text from Doctor: To Health Service, Case Western Reserve University RE: Injury/condition of patient X Please be advised that X is a patient of mine who is being treated for Y. I have advised X to [limit walking, use crutches etc.] for x amount of time. If you have further questions please contact my office at XXX-XXX. 2) Fax the copy of your doctors explanation along with a typed note from you that specifies a) where you currently park (lot number) and, b) where you would like to park (lot number) for the duration of your injury/condition. Please name a first and second choice for closer parking as some lots are so limited as to be unable to accommodate a temporary change. Fax number for Health Services: 368-8530. This process is confidential and information about your injury/condition will remain in the Health Service Office. For questions about this process or HIPPA call Health Services at 368-2450. 3) Health Service will process a request for your temporary permit within 72 hours and submit it to Access Services. The temporary permit is usually issued within 48 hours. Access Services will call you when the pass is ready, but if you would like to check on the status of your temporary permit please call 368-2273. Access Services will work to meet all requests, but all first choice requests may not be possible. If you have questions about permanent accommodation, please contact the Equal Opportunity and Diversity Office at 368-8877.
