All candidates applying for faculty positions at Case Western Reserve University must receive an email (where an email address is
available) requesting their participation in the collection of voluntary affirmative action data.
A sample email might read:

Reserve University. Thank you for your interest in our department.

http://eodsurvey.case.edu/aa/affirmative_action.htm. Completion of this
survey is optional and will not affect the status of your application. The information collected is confidential and goes directly to the Faculty Diversity Officer, who reports aggregate data annually.
