This form must be submitted by Friday, April 17 at midnight.

 

Please, fill a separate form out for each piece you perform (there are a few groups with more than one), because we may split up a single group's pieces in the show in order to maintain variety and a high level of audience entertainment.

 

Several parts of this form are for printing in the program or time planning. Please be sure the information you submit is accurate. Programs will be printed on April 17, so any changes MUST be submitted by then for inclusion in the program. Email Sarah with any changes or concerns about information on this form.
