Thank you for joining us to present a workshop at the 2011 Northeast Ohio Practice-Based Research Festival on December 3, 2011, 8am to 4pm at the Embassy Suites in Independence, Ohio.  

Please provide us with PowerPoint copies of

• Research Project Presentation (10 minutes)
• Sampler presentation  (3-5 minutes)
• Workshop presentation

Via email as soon as possible, or by Thursday, December 1, 2011, at the latest. 

• The presentations will be available online for attendees to access after the Festival. If you would not like your entire presentation to be available online, please provide us with an online version to offer attendees. 
• You can find generic PowerPoint templates for the Sampler and Workshop Presentations below, for your convenience. 

We will be including a biography for each presenter in the program.

• Please complete this form to provide a 1-2 paragraph Biography for the program by 11/17/2011. You may come up with a new Bio or forward one that you have used previously, including a sending me a link to an online Bio that you endorse and that I can copy and paste into our program.
