Mission Statement
To develop a comprehensive algorithm to guide clinicians through an individualized nutrition care process as it relates to patients with chronic kidney disease.
Objectives
The objective of the Nutrition and CKD Project is to develop a tool that will:
- Identify nutritional abnormalities in patients with CKD
- Recommend specific nutrition assessment parameters tailored to the patient's identified nutrition abnormalities
- Identify nutrition diagnosis(es) and etiology(ies)
- Provide clinicians structure in choosing intervention(s) based on best practice recommendations
- Provide structure for evaluation and monitoring of specified outcome(s) and determine need for additional intervention
