The objective of the Nutrition and CKD Project is to develop a tool that will:
- Identify nutritional abnormalities in patients with CKD
- Recommend specific nutrition assessment parameters tailored to the patient's identified nutrition abnormalities
- Identify nutrition diagnosis(es) and etiology(ies)
- Provide clinicians structure in choosing intervention(s) based on best practice recommendations
- Provide structure for evaluation and monitoring of specified outcome(s) and determine need for additional intervention
