The program accepts the standardized Dietetic Internship Program Application (Revised July 2005) and Recommendation Form (2004) developed by Dietetic Educators of Practitioners.

Applicants are screened for eligibility for admission to CWRU’s School of Graduate Studies. Candidates are then evaluated on:
- Academic Achievement
a. 3.0 in major, science classes and overall GPA
- Work and/or Volunteer Experience
- Letter of Application – typed, no longer than two pages and should include:
a. Your reasons for entering the dietetic profession.
b. How previous work/or volunteer experiences have helped to prepare you for the future.
c. Goals upon completion of the program.
d. Reasons for choosing this program.
e. Special qualifications for this program that distinguish you from the other applicants.
- References
a. Three references from at least two professors in major field or study, preferably using the form included with the Supervised Practice Program Application prepared by ADA.

An official transcript from every university or college attended.

Declaration of Intent to complete degree and didactic program in dietetics requirements or verification statement of completion of didactic programs in dietetic academic requirement. These forms are provided and signed by your didactic dietetic program’s director.
- Application Fee of $30.00. Please make check payable to:
Department of Nutrition Services UHCMC
- Completed application must be postmarked by the February deadline.
- If an application is not complete, an explanation must be provided. An incomplete or falsified application will not be considered.
- Photocopied materials are acceptable, however all signatures must be original.
- GRE Scores are not required for entrance into the program.
