Keyshop Key Request Form



Beginning April 5, 2004, the Keyshop will begin accepting key requests from the online PDF form. The goal of this new requesting method is to stream line the key request process. All key requests by June 1st, 2004 will need to be submitted in the new format. Access Services and the BRB security desk will no longer accept the old request forms after that date.
  • The form should be completed, printed, then faxed to x0791.
  • Each Request form allows for three keys to be requested for an individual.
  • The form will then be checked for errors and/or completeness and forwarded to the Key Shop.
  • If the request is not valid, notifications will be made via e-mail immediately.
  • Once the key has been cut the requestor and the authorizing person will be notified via e-mail that the key is ready and can be picked up at Access Services or the B.R.B. Security Office. All keys requiring payment will need to be picked up at Access Services.
  • Note: Keys being requested in the Medical School should be taken to the Medical School Facilities office (T201) for approval.
  • The key will ONLY be available for pick-up for FORTY-FIVE (45) days after the date of the e-mail notification.
  • All Undergraduates and Non-Case requests must have a letter of authorization from the department. This letter can be e-mailed to keyshop@case.edu or faxed to 368-0791.

    All Lost Key requests must have a letter of authorization from the department. This letter can be e-mailed to keyshop@case.edu or faxed to 368-0791. There will be a $50.00 fine for lost key per replacement paid by the person who lost the key and $75.00 fine per key charged to the department when a person leaves and the key is not returned.
