Issuance
Case Western Reserve University will issue an identification card to all faculty, staff, undergraduate students, graduate students, Post Doctorate Fellows, professional students, cultural exchange persons, emeriti, and retirees of the University. One card only will be issued per person and the status on the card will correspond to the person's primary affiliation with the University. The identification card issued to the aforementioned entities will entitle the authorized cardholder to all of the privileges offered by the University as defined in the University policies, guidelines, and related documents.
Fees
The initial identification card issued to these persons will be produced free of charge. Replacement cards due to loss or damage including holes punched in cards, will reissued at a cost to the cardholder. Identification cards which exhibit deterioration from use or which are stolen from a secured area (with a report number issued by a law enforcement agency) will be reissued to the cardholder at no charge. Persons leaving their cards unattended and unsecured which results in theft of the card will be assessed the current replacement fee .Issuance and replacement costs will be determined each year by Access Services and will be published in Campus News at the beginning of each fiscal year.
Termination/Change of Status
The identification card must be returned to Access Services upon termination of enrollment, employment or other affiliation with the University. Persons remaining affiliated with the University but changing status must return their identification card promptly for reissuance in their new status classification. The university is not responsible for any loss or expense resulting form the loss, theft, or misuse of the identification card. The card is not transferable and must be presented upon request to establish official University identification.
Affiliated Institutional Groups
Identification cards will be issued to members of affiliated institutions and entities including but not limited to the Cleveland Institute of Art, Cleveland Institute of Music, English Language Studies, temporary University employees, contractors, and attendees to summer conference programs on campus. Affiliated status will be determined by approval of the University Administration. The identification card assigned to these individuals will be designated appropriately and will be limited to the duration of their affiliation with the University. Privileges assigned will be issued on an as needed basis. An issuance charge will be assessed for the production of the affiliated identification cards either to the individual or to the sponsoring host at the time of issuance.
