2009 Benelect Guide - Vision
The purpose of this guide (print pdf version) is to provide you with an overview of Benelect—the flexible benefits program at Case. This is not intended to be a comprehensive description of the benefit plans. Details of individual benefit plans are provided in legal plan documents and contracts that govern the operation of the program. Specific coverage information is contained in the individual summary plan descriptions available from the medical insurance carrier or Benefits Administration. Employees are responsible for selecting and using their benefits prudently and in the most cost-effective manner.
VISION COVERAGE
Eyecare benefits provide you and your family with access to quality vision care. Through Benelect, you have two plans from which to choose. Vision enrollment is open only in even-numbered years and is a two-year election. In other words, your coverage choice remains effective from January 1, 2008, through December 31, 2009. However, depending on a qualifying change of event, you may increase or decrease coverage or opt into one of the plans at any time if your spouse (equivalent) loses employer-provided dental. And, of course, new hires in odd-numbered years may elect vision coverage at their time of orientation.
VSP
For half a century, VSP has been the leader in eyecare wellness benefits. VSP has a proud tradition of providing quality eyecare coverage to employees. Union Eye Care
Union Eye Care Center, Inc. and University Hospitals of Cleveland are offering a vision plan benefit. The plan provides for an annual eye examination, lenses and frame or contact lenses through any Union Eye Care location. Eye examinations are available through doctors at Union Eye Care, University Ophthalmologists, Inc., University Eye Care and Surgery, Pediatric Ophthalmology. Ophthalmic Consultants of Cleveland, and University Ophthalmology Associates. The vison plan provides for a benefit plus discounts of 25 to 45 percent for eyeglasses and discounts of 10 to 20 percent for contact lenses through network providers.
Vision Coverage Level
As with medical and dental, you have coverage categories from which to choose:
- Employee
- Employee + Child(ren)
- Employee + Spouse (Equivalent)
- Employee + Family
Waive Vision Coverage
If you already have eyecare or do not want vision coverage, another option is "waive."
