It's easy to get started with your Charity Choice 2009 contribution in one of two ways.

PDF version available - If you would prefer not to contribute online, a printable version of the form is available here. Simply print the form and send it to:

Contribute Online - First, read the directions below (full, printable instructions can be found here). Then at the bottom of this page, click on the 'Pledge Now' button. You will be taken to the PeopleSoft form where you will declare the amount of your contribution and the method of payment.

1. Login to the Case HCM System using your Case ID and password.
2. Click on "Add Contribution"
3. Use the drop down box to select any specified Federation Name or Independent 501(c)(3) to donate to a charity not listed. If choosing a federation, you may designate a member agency if you wish.
4. Select a Donation Type:
   • Payroll Deduction - Enter all the fields; click tab from one field to the next.
     Number of Months is set to 12. If you choose to change that field, you must tab to see the new calculated total.
   • Check - Make check(s) payable to the Federation (United Way, Community Shares, Earth Shares) or Independent 501(c)(3) agancy of your choice. Send check(s) to:

5. Click "Save"
6. Click "OK" to view your contribution. Be sure to print a copy for your records.
