REGISTRATION OF SUPERVISION
The State Board of Psychology requires that all
training and work supervision be registered with the board. Supervision
of activities approved by the clinical committee (e.g. practicum, field
placement) is considered training supervision and the program coordinates the
reporting of the required information to the board.
Who is your supervisor?
The supervisor is the licensed psychologist who accepts the responsibility
for your activities as a trainee. This may not be the person who is
designated as your field placement supervisor by the program for administrative
purposes. You must determine who your actual field placement
supervisor(s) is(are). Your practicum supervisor must also be reported to
the board unless the supervisor is a full-time member of the faculty of
the Department of Psychology. Your supervisor may be supervising
other people, of course, and there is a limit to the number of supervisees who
may be listed on the license of any one psychologist. This limit is 4 but
refers only to work supervision, not to training supervision. You
should know that if this comes up at your placement.
What must be reported?
The name of the supervisor, license number and starting date of supervision
must be reported to the Board.
The date of termination of supervision must also be
reported to the board.
What if
my supervisors change?
Changes
must be reported to the DCT for transmittal to the Board on the UPDATED
SUPERVISION REGISTRATION FORM as they occur.
What must you do to register supervision?
SECOND YEAR STUDENTS must complete
the STATE BOARD OF PSYCHOLOGY
SUPERVISEE REGISTRATION FORM (SRF). For information about how to

This form must be submitted to the DCT by October 1 for his signature and
transmittal to the Board of Psychology. Please include your SSN; it
will make the required reporting in subsequent years much easier.
THIRD AND LATER-YEAR STUDENTS
By October 1 of each year, an UPDATED
SUPERVISION REGISTRATION FORM must be submitted to the DCT.
This form will be used to report new supervisors
and the termination of previous supervisors.
STUDENTS GOING OFF ON INTERNSHIP
Unless the DCT is notified otherwise, termination of all supervision will
be reported at the start of the new academic fiscal year, July 1.
