1. The applicant must complete the following application - the application may be submitted electronically to carlos.crespo@case.edu or sent to:

2. Please arrange for a recommendation letter from a science (preferably chemistry) teacher who knows you and your academic abilities. Details for the teacher are given at the end of the form.


3. We require a copy of your most recent official high school transcript. Have it sent to:
