Enrollment Forms

1. You will need to fill out two forms:

   • Alumni Audit Enrollment Data Form

   • Course Selection Form

   If you cannot download these, request these forms by calling 216-368-4413. Ask for the for the Alumni Audit enrollment packet. It will be mailed to you.
   

2. Fill out forms. Be sure to sign the Course Selection Form. See Important Dates for submission deadline.
